The following items are required: Submission of completed documents and initial fees, including  (1) New Business License Application  (2) SAVE Affidavit (must be notarized)  (3) E-Verify Affidavit (must be notarized)  (4) Regulatory permits (if applicable)  (5) Copy of a valid government issued identification (i.e. driver’s license, passport)  (6) $75 non-refundable registration fee*  (7) $50 non-refundable zoning review fee** Payment must be made in guaranteed funds (i.e. money order, cashier’s check, etc.). Personal  checks are not accepted. Links to blank business tax forms are located at the bottom of the City of Atlanta Office of Revenue web page (click link below). Upon completion of processing and receipt of tax invoice payment, an official business license certificate will be mailed. License should be delivered within 7-10 business days after payment.
