To submit an online request, click the Create Request button below. Complete all of the fields, including:  (1) Your Name  (2) Email Address  (3) Phone Number  (4) Business Address  In the Customer Notes box, include  (5) Business Name  (6) Business License Number (tax account number)  (7) Other pertinent details that may assist us in reviewing the request Utilize the Attach New File button to upload:  (1) Completed Amendment form  (2) Copy of valid government issued identification (i.e. passport, driver’s license)  (3) Federal and State tax returns for the applicable prior year(s)  (4) Gross receipts for the applicable prior year(s)Submissions may also be done in person or by mail.
