Complete the online form with the following details: (1) Full name(2) Service address(3) Email address;Insert in the Summary of Request box: (4) Last four (4) digits of your Social Security Number or Tax I.D. Number (5) Specific request including account number Your request will be reviewed during normal business hours. If additional account verification is required, the water/sewer account holder will be notified.
