A plumber’s invoice and/or other supporting documents regarding the completed repair work should be included with the request. Please provide all related account number(s), including residential, commercial, and irrigation accounts. Complete and upload an Account Review Request form for each applicable account for which an adjustment is being requested. NOTE: Toilet leaks, running toilets, and leaking or broken faucets are excluded unless certified medical documentation for hearing impairment is provided. Customers are responsible for repairing leaks on private property.  To submit an online request, click the Create Request button below. Complete all fields, including: (1) Full Name(2) Email Address(3) Phone Number(4) Service Address In the Customer Notes box, include (5) Account Number(s)(6) Last four digits of social security number or tax ID(7) Other pertinent details that may assist us in reviewing the request.Utilize the Attach New File button to upload: (1) Completed Account Review Request form (2) Plumber’s invoice and/or other supporting documents regarding the completed repair work(3) Payment receipt (i.e. copy of canceled check, credit card payment receipt)Submission may also be done by fax, (404) 221-9518, or mail/in person delivery to the address stipulated on the form.
