Homeless letter dated within 7-10 days of appointment
Valid Photo ID
Social Security Card
Birth Certificate
Food Stamps Award Letter (if applicable)
Work History from the Department of Labor
Proof of income –
– Employment (6-8 consecutive check stubs, payroll history of 2-3 consecutive months)
– Social Security Award Letter
– General Assistance Award Letter
– Contributions from family
– Child Support Award document
