A request for a business tax refund requires completion of the Business Tax Refund Request Application form (see attachment). The completed form and supporting documentation must be submitted via email to refunds@atlantaga.gov.

In addition to the completed Refund Request Application, the following items are required:

• Copy of valid government issued identification (i.e. passport, driver’s license)
• Copy of front and back of cancelled check or other proof of payment

NOTE: Full tax returns (Federal and State) for years refund is requested may be required.
