A request for a business tax refund requires completion of the Business Tax Refund Request Application form, available for download HERE. The completed form and supporting documentation must be submitted via email to refunds@atlantaga.gov.
In addition to the completed Refund Request Application, the following items are required:
Copy of valid government issued identification (i.e. passport, driver’s license)
Copy of front and back of cancelled check or other proof of payment
Full tax returns (Federal and State) for years refund is requested
