Account holders who wish to disconnect water/sewer accounts and transfer services to a new address, must:  

  1. Download and submit the required Disconnect Request form (see link below).
  2. Follow instructions below under section titled “Transfer Water And Sewer Services To a New Address”

Methods for Submitting Disconnection Request Form:

If the account holder is deceased, provide a copy of the account holder's death certificate with seal, and a copy of the water and sewer invoice. NOTE: Last of testimony papers or new probate court documentation is required if you are trying to establish service. The water and sewer account will not be closed until these documents are received.

 

If moving inside the city, all balances must be paid in full before services can be transferred.

Failure to disconnect water and sewer service when vacating a property may result in liability for water and sewer charges.

 

ATTENTION: DWM does not perform same day disconnections. Disconnection must be scheduled on Monday, Tuesday, and Wednesday only.

*Requests for disconnection of water and sewer service should not be submitted more than 30 calendar days in advance*

 

Transfer Water And Sewer Services To a New Address                                                                             

Existing account holders who wish to transfer services to a new address must complete and submit a new water/sewer service application (See link below).

Requirements:

  • Completed Water/Sewer Service Application.
  • Copy of lease agreement (1st page and signature page), closing statement, closing disclosure (HUD), settlement statement (HUD-1), or warranty deed, including the physical address of the property within the body of the document or tax information. Businesses must also provide IRS form SS-4 with EIN number.
  •  A legible copy of government-issued identification in the applicant(s) name(s), such as a valid driver's license or passport.
  • A letter of authorization must be provided if establishing service on behalf of a business.
  • Account establishment fee of $15.00.
  • Account deposits are required and will be based on the size of the meter located at the service address.
    • Deposit amounts are provided once an account is fully established.
    • Deposits are billed on the account’s first billing statement.
