The Housing Opportunities for Persons with AIDS (HOPWA) Program is the only Federal program dedicated to the housing needs of people living with HIV/AIDS. The HOPWA program, run by the U.S. Department of Housing and Urban Development (HUD), provides housing assistance and support services to low-income individuals and families living with HIV/AIDS. The goal is to prevent homelessness and improve their quality of life by providing housing and the support needed to live healthier, more stable lives.
Supportive Services include a variety of assistance programs designed to help you manage your health, maintain stable housing, and enhance your well-being. These services address different aspects of your life to ensure you have the support you need through Case Management. A case manager works with you to create a personalized plan addressing your specific needs and goals. The case manager will help you access healthcare, housing, benefits and other necessary services.
Case manager will provide ongoing support and adjust your plan as needed. Regular check-ins are conducted to ensure you are receiving the necessary assistance and progressing toward your goals.
