Professionals may elect to have taxes based on a flat tax rate of $400.00 (professional registration) or on gross receipts and number of employees (licensed general business).NOTE: ONLINE PAYMENTS ARE NOT AVAILABLE FOR THIS REQUESTNew professional registration requires the following: (1)  Completed Professional Registration form (2)  Copy of State of Georgia professional license(3)  Completed SAVE Affidavit (must be notarized)(4)  Copy of valid government issued identification (i.e. driver’s license, passport)(5)  $400 non-refundable registration fee(6)  $50 non-refundable zoning review feeLinks to blank business tax forms are located at the bottom of the City of Atlanta Office of Revenue web page (click link below).Professional registrations are automatically renewed and billed. (See NOTE below)• Non-Attorney payment is due April 1.• Attorney payment is due June 1. The penalty for late payment is 10% of the tax due. Interest accrues at a rate of 1.5% per month. NOTE: A new notarized SAVE affidavit, and copy of a valid government issued ID, must be provided for renewals if there is a change from the previous year. Ex: Previous year affidavit signed by someone who is no longer with the firm.
