solution home » services » solution Business Failure to File Multiple Year RenewalA Business Tax Multi-Year Renewal Application must be completed if the business tax account was not renewed by December 31 or a notification regarding multiple year failure to file has been received. The application must be submitted in person or by mail. A $50 zoning review fee will apply if the business physical location has changed. Submission of the following documents and fee is required:  (1) Completed Business Tax Multi-Year Renewal Application  (2) SAVE Affidavit (must be notarized)  (3) E-Verify Affidavit (must be notarized)  (4) Copy of a valid government issued identification (i.e. passport or driver license)  (5) $50 zoning review fee (applicable if business physical location has changed)Once the application review is successfully completed, you will receive an invoice from the Department of Finance, Office of Revenue, for applicable business taxes.
