The following items are required:  (1) Completed Non-Profit License Application  (2) Completed New Business License Application  (3) Copy of the organization’s State Charter, if a Georgia based Corporation  (4) IRS Form 501C3 which affirms the organization’s tax-exempt status (i.e. military veterans,   church/charitable organizations)  (5) Veteran identification card or Department of Veterans Affairs letterhead that affirms 30 percent plus   disabled veteran status, if applicable  (6) SAVE Affidavit (must be notarized)  (7) E-Verify Affidavit (must be notarized)  (8) Copy of valid government issued identification (i.e. passport, driver’s license)  (9) $50 non-refundable zoning review fee** Payment must be made in guaranteed funds (i.e. money order, cashier’s check, etc.).
