The form must be attached to the online request. A request to change the physical location of the business, within Atlanta city limits, requires completion of the Change of Address form, zoning review and approval. A $50 fee is applicable for the zoning review (guaranteed funds only). The form and payment must be submitted in person or by mail. Links to blank business tax forms are located at the bottom of the City of Atlanta Office of Revenue web page (click link below). To submit an online request, click the Create Request button below. Complete all of the fields, including:  (1) Your Name  (2) Email Address  (3) Phone Number  (4) Business Address  In the Customer Notes box, include  (5) Business Name  (6) Business License Number (tax account number)  (7) Other pertinent details that may assist us in reviewing the request Utilize the Attach New File button to upload: (1) Completed Change of Address form (2) Copy of a valid government issued identification (i.e. passport or driver license)Submissions may also be done in person, to expedite the request, or by mail, which will extend processing of the request.
